Boarding Consent Form

Ox Bottom Animal HospitalOx Bottom Animal Hospital cannot guarantee against accident, and we cannot be liable for loss or damage caused by or to our pet guests at this facility. Ox Bottom Animal Hospital charges for boarding space by the night. Owner agrees to pay the boarding rate that is in effect on the day that the pet is checked into the facility. Payment for boarding is due upon checkout.

Check-in/out Times: Check-in times and check-out times are during normal appointment hours: 7:00 AM - 6:00 PM Monday - Friday. Saturday and Sunday pick up is available for weekends between 5:00pm - 6:00pm and must be paid the following Monday. Owners that pick up their pets on Saturday or Sunday, will pay for a half-day. There are no Saturday or Sunday check-ins. If your pet is due for updates while they are boarding on a Saturday or Sunday, it is the responsibility of the owner to update the pet prior to dropping off Friday. If you want our facility to update your pet, do not drop off later than Friday at 12pm, or schedule an in-room appointment prior to boarding.

List of approved Flea Prevention: Bravecto, Nexgard, Simparica, Trifexis, Credelio, Revolution, Cheristin, and Comfortis

Departure Baths: We require bathing services for all boarded dogs. Bath charges depend on the size of your dog, and earn a 10% discount off the bath price per each night of boarding.

I hereby agree to the following as the owner of the aforementioned pet.

I further certify that my pet is in good health and has not been ill with any communicable condition nor to my knowledge been exposed to any communicable diseases within the last 30 days. Moreover, I certify to the accuracy of all information given about my pet and have discussed any previous signs of aggression or threatening behavior toward any person or animal. I have read and understand the entire boarding consent form.
